If you are a healthcare professional who has already assessed a patient and their medication for suitability for a Compliance Aid device please fill out the form below and a member of our team will be in touch.  PLEASE NOTE COMPLIANCE AIDS ARE FOR PATIENTS ON THREE OR MORE REGULAR MEDICINES DAILY.   PLEASE ONLY COMPLETE THIS FORM IF THE COMPLIANCE AID WILL HAVE MORE THAN 3 DIFFERENT MEDICINES IN IT.  

If an assessment has not already been completed by a healthcare professional please fill in this form instead.

Please note we require 7 working days for a compliance aid device to be ready for collection or delivery.
